The way it works right now, it kicks the newest spectator regardless of how they became spectator. They could have joined after the game started or they could have just been killed in the match. I think this should be changed so tributes who are killed are not added to the end of that list. It...

Yup, they must have got it first. Your client thinks you got it until the server checks and tells it that you didn't, so your client updates it and the items disappear from your inv. No real way to fix this unfortunately.

That's exactly how the system is set up. At the beginning of each round, a map is randomly picked from the list of available maps.

I believe it is a bug that the door is already open. It should be reset by the plugin at the beginning of each match.
