Approximately how many tributes and spectators: 1 tribute, 1 or 2 spectators
Time of day: 7PM - 2AM PST
What happened: When I killed the last person it said there was another person so the games didn't end.
Is it reproducible: Of course!
How severe is it: Very severe it prevents the win and if there is a lot of time left the only way to get a quick end is to make suicide.
