When you switch items in your hotbar, it comes up with the name of an item above your hotbar. (Then fades away.)

Well, this has never happened to me, and it still won't work.

It is not texture pack I am using, so I REALLY, REALLY need some help.

I don't think optifine glitches this however i think its just you options in the normal mc menu.

I can see you are having trouble.
First options. Animations if not there quality or where you find better grass and clear water option. In. There you will find item tool tips. Turn the off. Sorry this wasn't very helpful I just wanted to tell you. other question? Shoot me a pm
