On the page of mod info it'll say like Age: Name:. Where would you put that? Or do you incorporate it into your mod app.

You make a new thread for yourself with your application. It should be placed in the page that my second link leads to. Name your thread "Username - Moderator Application." Replacing the word username with your actual username.

Oh, you don't have to worry about that. Both "Make a new thread" buttons place the thread in the same place.
